To be re-credentialled, you will need the following forms to demonstrate your continuing competence
NB Cytological statistic summaries covering the whole 3 year period since your last credential must be submitted along with the relevant forms below. If the laboratory cannot generate your statistics in your name you can use the Cytological Stats Record to keep records of all your stats to submit.
If you work in an organisation that is accredited, you need to complete these two forms
If you work in a non-accredited organisation you will need to submit the following three documents
Credential - a predetermined set of standards, establishing that a person has achieved professional recognition in a specific field of health care.

Practice Nurses Taking Pap Tests
MBS ITEMs (10998 & 10999) allow for Pap tests taken by a practice nurse in a regional rural or remote area. Practice nurses undertaking Pap tests must be appropriately qualified and trained - which includes being credentialled in states where this is available. All Practice Nurses undertaking Pap tests must also have completed an accredited course.
